Participant Process

Interested parties fill out eligibility questionnaire (link below)

Should participants meet eligibility criterion, they will be sent a welcome email with two (2) intake questionnaires. Participants must complete intake questionnaires before Module 1 to qualify for thank you package.

Upon completion of intake questionnaires, participants will gain access to educational modules on a weekly basis.

After completion of educational modules participants will be emailed the same questionnaires in order to collect feedback. Participants must complete feedback questionnaires and submit to qualify for thank you package
